Part 6 – Cholesterol, Diabetes & Metabolic Syndrome
Understanding the Hidden Risk Factors That Can Threaten Your Heart

Introduction
In the first article of this series, we explored how the heart functions as the body’s life-sustaining pump. Part 2 examined coronary artery disease (CAD), while Part 3 focused on high blood pressure—the “Silent Killer.” Part 4 explored heart rhythm disorders, including atrial fibrillation and modern treatments. In Part 5, we examined another group of interconnected conditions—high cholesterol, diabetes, obesity, fatty liver disease and metabolic syndrome—that can quietly increase cardiovascular risk. We discussed how these conditions can accelerate atherosclerosis and increase the risk of heart attack and stroke, as well as the importance of lifestyle changes, knowing your numbers and appropriate medical treatment. In this sixth installment, we take a closer look at how these metabolic risk factors affect the cardiovascular system and what can be done to protect the heart through prevention, early detection and comprehensive care.
Understanding Cholesterol: Good vs. Bad
Cholesterol is a waxy, fat-like substance that the body needs to build cells and produce hormones. It travels through the bloodstream inside particles called lipoproteins. Low-density lipoprotein (LDL) is commonly called “bad cholesterol” because elevated LDL can contribute to plaque buildup inside artery walls. Over time, this process, called atherosclerosis, can narrow arteries and increase the risk of heart attack and stroke. High-density lipoprotein (HDL) is often called “good cholesterol” because it helps transport cholesterol toward the liver for processing. However, cardiovascular risk cannot be determined by HDL alone; LDL and a person’s overall risk profile remain central to treatment decisions.
Triglycerides: Another Important Number
Triglycerides are another type of fat found in the blood. The body uses them for energy, but elevated levels are often associated with obesity, diabetes, metabolic syndrome, excessive alcohol consumption and diets high in added sugars and refined carbohydrates. Very high triglycerides can increase the risk of acute pancreatitis. When high triglycerides occur together with high LDL, low HDL, high blood pressure or abnormal blood sugar, cardiovascular risk may rise substantially.
Diabetes and Heart Disease
Diabetes is one of the most important risk factors for cardiovascular disease. Persistently elevated blood glucose can damage blood vessels and promote inflammation and atherosclerosis. People with diabetes have increased risks of coronary artery disease, heart attack, stroke, peripheral artery disease and heart failure. Type 2 diabetes is strongly associated with obesity, physical inactivity and metabolic abnormalities. Good blood-sugar control, along with management of blood pressure and cholesterol, can significantly reduce cardiovascular risk. Some newer diabetes medications, including SGLT2 inhibitors and GLP-1 receptor agonists, also provide cardiovascular benefits for appropriate patients.
Obesity and the Heart
Excess body weight can affect the cardiovascular system in several ways. Obesity can contribute to high blood pressure, abnormal cholesterol, diabetes, sleep apnea, inflammation and fatty liver disease. It can also increase the workload on the heart and raise the risk of heart failure and atrial fibrillation. Even modest, sustained weight loss can improve blood pressure, blood sugar and metabolic health. The goal should be better overall health and reduced cardiovascular risk—not simply a number on the scale.
Fatty Liver and Cardiovascular Risk
Metabolic dysfunction-associated steatotic liver disease (MASLD), formerly called nonalcoholic fatty liver disease, occurs when excess fat accumulates in the liver in association with metabolic risk factors. It is commonly linked with obesity, insulin resistance, type 2 diabetes and abnormal cholesterol. Although it primarily affects the liver, MASLD is also an important marker of metabolic and cardiovascular risk. Managing weight, blood sugar, blood pressure and cholesterol is therefore important for both liver and heart health.
Metabolic Syndrome: When Risk Factors Come Together
Metabolic syndrome is not a single disease but a combination of risk factors that tend to occur together. These include increased abdominal fat, elevated blood pressure, elevated blood sugar, high triglycerides and low HDL cholesterol. Having multiple components increases the risk of cardiovascular disease and type 2 diabetes. The diagnosis should be viewed as a warning—and an opportunity to intervene before serious disease develops.
Lifestyle Changes: The Foundation of Prevention
A sustainable heart-healthy lifestyle is one of the most powerful ways to address these interconnected risks. Emphasize vegetables, fruits, whole grains, legumes, nuts, seeds and minimally processed foods. Choose healthy protein sources and replace saturated fats with unsaturated fats when possible. Reduce added sugars, refined carbohydrates, highly processed foods and excessive sodium.
Regular physical activity is equally important. Most adults should aim for at least 150 minutes of moderate-intensity aerobic activity each week, together with muscle-strengthening activities on at least two days when appropriate. Avoid tobacco, limit alcohol, maintain healthy sleep habits and develop practical ways to manage chronic stress.
New Cholesterol-Lowering Therapies
For some patients, lifestyle changes alone are not sufficient to lower cholesterol to an appropriate level. Statins remain the foundation of treatment for many people at elevated cardiovascular risk. Depending on individual risk and response, physicians may also prescribe ezetimibe, PCSK9 inhibitors, bempedoic acid or other therapies. Treatment should be individualized because the appropriate LDL goal depends on a person’s cardiovascular history and overall risk. Patients should never stop or change cholesterol medication without consulting their healthcare professional.
Know Your Numbers—and Your Risk
Protecting your heart requires more than knowing one number. Know your blood pressure, cholesterol levels and, when appropriate, blood glucose or A1C, weight and waist circumference. If you have diabetes, obesity, fatty liver disease or several metabolic risk factors, discuss your overall cardiovascular risk with your physician. Early identification provides an opportunity to intervene before a heart attack or stroke becomes the first warning sign.
The Bottom Line
The metabolic conditions discussed in this series are closely connected and can create a pathway toward cardiovascular disease. But that pathway is not inevitable. Healthy food choices, regular exercise, weight management, appropriate medications and routine medical care can substantially reduce risk.
A healthy heart is not protected by one dramatic change; it is protected by thousands of small, consistent choices made over a lifetime.
